Welcome to Our Online and In-person Psychiatric Clinic for Children and Adults (Ages 2-99) –Medication Management Services

At our premier online and in-person psychiatric clinic, we specialize in comprehensive medication management for both children and adults, ensuring quality mental health care for individuals aged 2 to 99. Our dedicated team of Psychiatric Mental Health Nurse Practitioners (PMHNPs) is equipped to diagnose and treat a wide range of mental health conditions, including anxiety, ADHD, depression, bipolar disorder, mood disorders, schizophrenia, addiction, sleep disorders, migraines, and more. We proudly serve clients across Arizona, Iowa, and Wisconsin, providing accessible and effective mental health support.

A Dual Role: Teaching and Treatment at Alay Clinic

In addition to offering exceptional psychiatric care, Alay Clinic serves as a leading teaching facility for advanced practice registered nurses (APRNs). Our clinic not only provides outstanding treatment but also fosters the next generation of mental health professionals. APRNs pursuing certification as Psychiatric Mental Health Nurse Practitioners receive hands-on training and mentorship from our experienced providers and preceptors. This commitment to education ensures that we deliver the highest standard of care to our patients while nurturing future leaders in psychiatric nursing.

Step 1: Get The Information You Need

Understanding Your Treatment Options: Intake Appointments at Alay Health

It’s completely normal to have questions when exploring treatment options. Every new patient at Alay Health begins with an Intake Appointment with a qualified Psychiatric Mental Health Nurse Practitioner. This comprehensive 60-minute telehealth session allows us to gather essential information about your specific symptoms, previous medications, and medical history.

Following your initial appointment, subsequent sessions are typically 20-30 minutes long and are scheduled based on your preferences and provider recommendations. By the end of the Intake Appointment, you and your provider will work together to develop a personalized treatment plan tailored to your needs. If the Alay Health Team isn’t the right fit for you, we will strive to provide alternative options.

Step 2: Fill out New Patient Forms via our Intake Portal

Requesting your first appointment is simple. Go to our New Patient intake portal. Once we receive your information – including a photo of your insurance card, a staff member of our team will give you a call to get you set up as a new patient. Your intake appointment will then be confirmed, and you’ll receive a a link to create an account with our patient portal.

Step 3: Complete New Patient Paperwork

Before your first appointment, you will receive a link to create a secure password for our Patient Portal. To complete this process, you will need to provide your name and date of birth; the email address you received the link at will serve as your username. Through the Patient Portal, you can review essential consent forms and office policies.

If you need to print any forms, please visit our “Insurance, Forms, & Patient Portal” page to download them and return them to us promptly. Please note that if your forms are not completed and submitted at least 24 hours before your initial appointment, your appointment may be canceled. Ensuring all documentation is ready in advance helps us provide you with the best possible care.

Step 4: Attend Your Appointment

To ensure you never miss an appointment, we provide multiple reminders via email, phone call, and text message. 24 hours before your scheduled session, you’ll receive an email containing a unique link for your video appointment. This email serves as both a reminder and a gateway to your session. Our automated system tracks when these links are sent, ensuring timely delivery.

Approximately 10 minutes before your appointment starts, simply click the link or copy and paste it into your internet browser. Follow the on-screen instructions to enter your video session, where your clinician will join you at the scheduled time.

If you need assistance, please visit our Help page. We kindly ask for your patience, as providers may occasionally run behind schedule, similar to in-person appointments. If your clinician has not joined within 5-10 minutes of your start time, please contact our office immediately for support. Reporting any issues promptly ensures we can address them effectively.
